Hospice Nurse New

Pride Healthcare is assisting a top healthcare organizationto identify the right person to fill a Hospice RN opening in Jackson, Ohio.

Responsibilities:

• Provide direct patient care services to hospice patients and supportive care to the patient’s family
• Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care
• Case manages all assigned patient clinical records.
• Assures the highest quality of care is provided and documented in accordance with the Plan of Care.
• Performs the initial and on-going assessment of the patient.
• Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group.
• Making supervisory visits to the patient’s residence at least every 2 weeks to assess the provision of Aide’s services.

Requirements:

• 1+ years experience as an RN, either in acute care hospital, oncology, long-term care, hospice or home health.
• Valid OH or Multistate RN license
• Active AHA BLS
• Knowledge of terminally ill patients and their families along with understanding of hospice.
• Thorough knowledge of managed care principles & regulatory guidelines (i.e., Medicare, Medicaid, ACHC, and human resource management principles.)
• Must be able to effectively operate nursing assessment tools & technology.
• Must have and maintain transportation to be used for work as well as current automobile insurance and valid driver’s license.

Additional Details:

• Tentative start: 4/21
• 13 weeks with possibility of extension
• Schedule: Mon-Fri, 8a-5p
• Travel Pay: $2126 weekly gross = $880 weekly taxable + $1246 weekly stipend
• Local Pay: $45-48/hour
• Mileage reimbursement: $0.44/mile; home to first patient and last patient to home does not count into mileage.
